Two Different Natural Course of Hypermature Cataract: a Case of Phacolytic Glaucoma, a Case of Membranous Cataract

Abstract
Purpose: we are going to report various end stage of hypermature cataract through our two cases which showed phacolytic glaucoma and membranous cataract.

Methods: One patient, a 65-year-old man, who have developed phacolytic glaucoma manifesting pseudohypopyon with high intraocular pressure (51 mmHg) in the right eye was referred. Anterior chamber irrigation was carried next day to control intraocular pressure (IOP), after medical treatment with IOP lowering agent. The other patient, a 72-year-old man, presented silent membranous cataract occurring after complete absorption of crystalline lens in the left eye. The white capsular membrane of the left eye was removed with ocutome, and scleral fixation of intraocular lens (IOL) was carried for visual improvement.

Results: The intraocular pressure of the right eye in the former was lowered to 9 mmHg at postoperative 1 day. Histological analysis of resected anterior capsule revealed amorphous eosinophilic material with microcalcification and macrophage was detected in the cytospin of aqueous humor. The visual acuity of the left eye in the latter improved to 20/40 at postoperative 1 week and good centered IOL was observed.
